Reporting of Claim by Employer 42-19-10, R. 67-411
An injury for which there is no compensable lost time or permanency and the medical treatment does not exceed $500 must be reported annually on a form and at a time prescribed by the commission.
An injury involving compensable lost time, medical attention in excess of $500, or the possibility of permanency must be reported to the employer's representative within 10 business days after the occurrence and knowledge of it on Form 12A or in an electronic format prescribed by the commission.

The employer must make a record of all work-related injuries reported by its employees on the Form 12A and retain the record for a period of 2 years.
