Reporting of Claim by Employer Minn. Stat. 176.231

Where death or serious injury occurs, the employer must report the injury or death to the commissioner and insurer within 48 hours after its occurrence. Where an injury has once been reported but subsequently death ensues, the employer must report the death to the commissioner and insurer within 48 hours after the employer receives notice of this fact. The employer may make an initial report by telephone, or personal notice, and file a written report of the injury within 7 days from its occurrence.

Where any other injury occurs which wholly or partly incapacitates the employee from performing labor or service for more than 3 calendar days, the employer must report the injury to the insurer on form FROI (First Report of Injury) within 10 days from its occurrence.

The employer must give the employee the "Minnesota Workers' Compensation System Employee Information Sheet" at the time the employee is given a copy of the first report of injury.

Filed within 14 days, if liability accepted can later deny with amended NOPLD within 60 days (M.S. 176.221)
